Photo, Video and Media Release *

• I understand that photographs, video recordings, and audio recordings may be taken during volunteer activities and events. By volunteering, I grant Sports for Disabled Vets Corp permission to use my image, likeness, voice, and statements in photographs, videos, publications, social media, websites, promotional materials, and other media without compensation, unless I notify the organization in writing before the event.

Assumption of Risk, Liability Waiver and Personal Property *

• I understand that volunteering may involve physical activity and other inherent risks. I voluntarily assume all risks associated with my participation. To the fullest extent permitted by law, I release and hold harmless Sports for Disabled Vets Corp, its officers, directors, volunteers, sponsors, partners, and affiliates from any claims, liabilities, injuries, damages, or losses arising from my participation. I agree to follow all safety instructions and immediately report any injury or unsafe condition. I understand that I am responsible for my own personal property while participating in volunteer activities or events. Sports for Disabled Vets Corp, its officers, directors, volunteers, sponsors, partners, and affiliates are not responsible for the loss, theft, or damage of personal belongings, including but not limited to vehicles, electronics, clothing, equipment, or other personal items. I acknowledge that I participate at my own risk and accept responsibility for safeguarding my personal property.

• I understand that certain volunteer positions may require a criminal background check to help ensure the safety and well-being of participants, volunteers, staff, and the community. By submitting this application, I acknowledge that Sports for Disabled Vets Corp may request my authorization to conduct a background check if required for the position I am seeking.
